Rules of Play:

1. All tickets will only be sold to persons 18 years of age and older.

2. Purchaser may be from out of province, however, must be in the province of Ontario at time of purchase.

3. Tickets can be purchased for:

• $10 for five (5) chances to win.
• $20 for 30 chances to win
• $50 for 150 chances to win
• $75 for 300 chances to win

4. Tickets will be sold using an electronic raffle system supplied by RaffleBox.

5. Tickets are available to purchase online at www.rafflebox.ca/raffle/lacghfoundation (www.lacghfoundation5050.com) with a valid credit card or in-person at the LACGH Foundation office located inside the hospital at 8 Richmond Park Drive, Napanee, ON, with cash or interact debit processed with an in-venue sales device.

6. After purchasing, patrons will receive a confirmation e-mailed with their 50/50 registered numbers from a Random Selection System. No specific numbers can be given as numbers are generated randomly.

7. In-person purchases will receive their numbers as a printed confirmation or via email.

8. In-venue purchases will receive a printed ticket automatically as well as an email if an email address is provided at time of purchase.

9. In-venue purchasers are responsible for their tickets. The LACGH Foundation will not be responsible for lost or stolen tickets. The winning ticket number will be selected by a Random Number Generation (RGN) system from all raffle numbers sold for the draw.

10. The amount of the 50/50 will be visible at www.rafflebox.ca/raffle/lacghfoundation

11. The winning number will be announced on the Foundations social channels and website (www.lacghfoundation.com) and will be able to be viewed at www.rafflebox.ca/raffle/lacghfoundation.

12. All prizes to be drawn at the Lennox and Addington County General Hospital Foundation office located at 8 Richmond Park Drive, Napanee, ON at the time indicated.

13. The winner may claim their prize by contacting the Lennox and Addington County General Hospital Foundation at: 613-354-3301 ext. 400, or by email at foundation@lacgh.napanee.on.ca. The ticket may then be brought to the LACGH Foundation office located inside the Lennox & Addington County General Hospital at 8 Richmond Park Drive, Napanee, Ontario K7R 2Z4 at an agreed upon time for ticket verification. The winner will then be paid by cheque for the amount posted at the event for which they bought their ticket. Winners must have the original ticket and no copies. All payments will be made by cheque.

14. The winning ticket holder has 6 months in which to claim their prize from the date of the draw their ticket was purchased for.

15. The winner must provide their name, address, and phone number along with picture identification for our report sheet.

16. Once the certified winner is verified, and provides identification with photo ID, they will be given a cheque in the amount of the declared prize, which is at minimum, 50% of the total sales for that draw. If the winner cannot provide photo ID, then the winning proceeds will be held until such identification can be provided.

17. In the event that no winner comes forward, the winning stub will be kept in a secured location and held for 6 months. After that time period has elapsed, the prize will be donated to a charity other than the Lennox and Addington County General Hospital Foundation, with approval of the Alcohol and Gaming Commission of Ontario (AGCO).

18. The following individuals are not eligible to participate in the 50/50 draws:

a. Board of Directors and Employees of the Lennox and Addington County General Hospital Foundation, their spouses, or any individual who resides in the same residence as the aforementioned persons are not permitted to purchase tickets and are not eligible to win.

b. Persons under the age of 18

c. Intoxicated Persons

Exceptions to the above information would include such things as publicly available information - names and phone numbers printed in telephone directories. Business contact information would also be an exception.

Information is kept secure and confidential. Steps are also taken to make sure information is maintained to ensure it is not lost or destroyed.
